How might it be possible to add a row to be printed in _cm_listview_print_after or _before that spans multiple columns in the document rather than just placing data into the document column structure?

All you can do is add extra rows using the current layout, so you can only use the current set of columns. It is not possible to insert a single item of data that spans several columns.
