GET CASE FOR A WORK FLOW [message #4101]

I have created a workflow with start task as add X option(sht_x_option(add1)) which is having a separate database called suchit with all the tables and columns as of your test but when I'm running the task sht_x_option(add1) to create a x_option so that a case will be created but the insert sql for case is running for infinite times but no case is created.please tell me where i'm doing mistake my workflow is fully valid.
